I've personally tested mostly on I2V with prompts such as "nipple lick" and "nipple suck" when both actors are in position.

I've also got positive results on a static nude image with prompts such as "One person named X in the middle of the screen. Girl comes from left and start licking nipples of X" (where X is name of the character)

P.S.: I have not tried self breast licking! Tell me in the comments if it works for you.
