To achieve good results you have to use a specific base prompt:
A sexual act of a woman. [Breasts description] with a penis between them. [Face description to make face appear on the image]. The man's legs are visible. [Opitonal, sometimes helps: "This is a titfuck POV."]

With this base you can add hands position, background and other features and the model should still generate good results (you can see some examples in my post). You can use higher weight for more consistent results but it may lead to worse image quality.

Recommended LoRA weight: 1.0-1.2
