In order to make sure nipples won´t become chaos, it comes at a cost of overfitting. Meaning that even if you choose a realistic model, the generated image will be semi-realistic at best. For example it used to be completely unthinkable to generate any nipple piercing in chilloutmix without LoRA, now it is possible, but only semi-realistic.

(Civitai doesn't ask you to post an image to evaluate a model anymore, therefore your fetish won´t get exposed by giving reviews or clicking like. Just remember to clear browser history and logout your account regularly.)
