Ichijou Kaori from the manga Katekano! (かてかの！) by Ohtomo Takuji. Trained for 800 steps from a variety of manga panels. Haven't found any limits on the tests, but was trained on some NSFW so take precautions if needed. One issue is that the manga's color panels have her with both red and blue eyes so there's a choice there - switch red to blue if you want blue eyes.
