Photoshop : draw a random white shape on a layer above your last good generated image version. Put the layer at about 60%. Export that image. Inpaint the part of the image that has the white spot you drew on pants, panties, notebook, ANYthing. If your prompt is good AND your help the AI with already drawn pixels and a low enough denoise to not change much your image : then you can get cum anywhere (with this Lora or any other similar).

I didn't try this Lora yet. But the way I manage all cum Loras : generate your image, work on it until it's perfect (img2img/inpaint). Then an then only work on adding cum, with a very low denoising. Generate tons of versions and use Photoshop to slowly layer each bit of good looking cum. And re-feed each time the result to your inpaint to work on the latest version.
To force the generation on specific areas (a desk, a keyboard, a nose, a hand, sheets, anything really) simply draw in photoshop a white stain (pure white ultra simple, no need to be an artist) where you want it. Then put this layer to about 60% opacity. Export your image with this bad quality drawing to inpaint, adjust your prompt and generate also multiple versions until you get the good looking generation.
And so on...
A great image goes through a bit more processing than waiting randomly for a generation to do what you'd like to magically see appear : DRAW what you want, help the AI to understand what's in your mind, give it some extra pixels to understand. And it'll work.
P.S : Best cum Lora so far in my opinion (again I didn't try this one yet) is the Lora "facialized" by Nelimee
