Seems like this lora is undertrained. The mouth placement is unstable because of this but that can be controlled a bit with natural language. There's prompt bleeding on the bellymouth trigger, so you need to use "fat, belly" on the negative prompt, and change the trigger like this: (belly:0.2)mouth.

Use a 1.3 strength.

Update: I was able to solve the problem by not using the trigger and describing the features with natural language instead.
