Lora Strength: 0.25-1.0 (Stronger lora may give more detailed nipples but changes the original image a little bit more, while lower values will preserve the original generation even more, i usually start with 0.35)
No trigger world needed, just make sure that your prompt let the character with the nipples exposed.
