What dare I say is an ovary prolapse?
Fully prolapsed womb, with the ovaries hanging from fallopian tubes (pulled through). Leaves the ovaries dangling on the outside of the inverted womb.
Not particularly realistic, but that's not exactly surprising for extreme prolapse stuff.

Any plan to do an updated version with improved ovarian prolapses? They're kinda a niche thing, but there are images out there (sankaku, pixiv, and ex have decent amounts, although ex lacks a specific tag).
