TLDR: This is my first attempt to draw non-trivial interactions in an operating room. It is not perfect. It will generate bad compositions quite often. However, if you are patient enough, you can generate decent images, and even combine this with a character LoRA, to draw something that wasn't possible before.
If you have been drawing AI images with Stable Diffusion for a while, you are probably well aware of one of its major flaws - it handles multiple person interactions very, very poorly. It is almost impossible to draw non-trivial interactions of more than one character. For example, it is impossible to tell AI to draw "a nurse gently touches the shoulder of a patient in hospital gown", expecting it to draw the two persons, their outfits, and the interactions correctly. And if you want to combine this with two distinct character LoRAs? You would have to use more advance techniques like in paint.
Since I am interested in medical / surgery scenes, I have always hoped to be able to train a LoRA to draw surgeon / patient interactions. This LoRA draws a scene with a view from above, where the patient is lying on the operating table, partially naked, looking nervous. Two surgeons are standing by the operating table, looking upwards so their faces fully visible.
Known issues:
In portrait mode, you might get deformed patient bodies - like an upside down head, body too long / too short, or just some random artifacts.
The patient might or might not be naked. You can try putting "nude" or "breasts" / "nipples" in the prompt, but it can have the unwanted side effect to make the surgeons naked as well.
Sometimes the surgeons don't wear a mask.
The girls tend to look the same. Even more so if you use a character LoRA. Sorry, you have to fix it with inpaint.
Getting a good picture takes time and patience. I think half of the results are acceptable, depending on your luck and how picky you are.
If you are okay with all these limitations, give it a shot. It will unlock certain scenes that were not possible to draw before. Enjoy.

i like the idea, but why this weird pov?
maybe "first-person-pov" or "first-person-view" as a training prompt will be better?
If you want POV from the operating table, I already have a LoRA for that. The goal is to draw both the surgeons and the patient in the same frame.
@phageoussurgery439 but they are just looking up in the middle of the room. that's what i am critizicing in a good meant way.
@themancalledmojo999 Hmm. I still don't get what you want to see. This LoRA is meant for a top down view of the operating table. The patient is looking upwards for sure, and the surgeons also lift their faces so you can have a clear look. Do you prefer the surgeons to lower their heads and look at the patient instead?
@phageoussurgery439 yes. a more realistic surgery. why would surgeons look up in the middle of a surgery? and why is a patient awake in the middle of a surgery? that's the point.
sorrry. i really don't mean this in a bad way. just think of it for ways to change your model in a good way...
@themancalledmojo999 I see what you mean now. I am considering making a model that have surgeons heads down operating on the patient, but there are values in making this model. I got requests for drawing "character X and Y operates on Z". Okay, if they are heads down, you probably won't recognize what characters are out there. As of why the patient is awake... that's just my sadistic personal preference if you know what I mean... If you want the patient to be asleep, I have a LoRA for general anesthesia too.
