The effect can be visible simply by using a combination of tags known to the base model since these were part of the training. For example, using "from below, panties" is sufficient. The output depends on the tags to determine visibility. For example, tagging hair color will increase the visibility of the character's head. Same for "looking at viewer" or "looking down". Using "upskirt" or "extended upskirt" can help increase the effect.
Useful tags for visibility: [haircolor] hair, [eyecolor] eyes, looking down, looking at viewer, panties, bra, navel, pussy, ass, back, breasts, etc.
Recommended quality tags: score_9, score_8_up, score_7_up, score_6_up. Optional: source_anime
Recommended negative tags: score_4, score_5. Optional: 3d, lips, realistic

I'm having issues where the dress/skirt gets extended over the character's head as well. Is there a negative that's convenient to help stop this?
you mean covering the character's head? i.e. the head is not visible? in the model info I explain that you need to describe what you want to see visible, such as hair, eyes, facial expression, etc. also, using looking at viewer or looking down helps.
