Toudou Haruka from the Japan-Only horror game Kisei Joker, a game by Blue Sky!
It's basically a resident evil clone made with rpg maker, with resource management, multiple endings and all. Sadly it was never translated into english. Here's a playthrough if you're ever interested.

(I'm not sure what is her actual eye color. The overworld sprite had some shade of red while her dialogue portrait was mostly black, so I settled for black in her default and red for her mutated form. Her shoes can be anything you want.)
