If you want a boy under girl, you can try 1girl, 1boy instead.
But that didn't work well for me.

For large breasts add:

Large breasts, huge breasts, enormous breasts

With some further testing I've found out that using 1boy, 3girls and Lora weight 0.8, you will able to create images with 3 girls, try it out!
